Loyola Medi Van is coming to School District #87 
Loyola University Medical Center Mobile ClinicOffering Routine Physicals and ImmunizationsAugust 5, 2008 and August 14, 2008 from 10:00am - 2:00pm
The mobile clinic will be in the Northlake Middle School/Riley School parking lot
August 19th from 12:00pm - 4:00pm
The Mobile Clinic will return to the Northlake Middle / Riley School parking lot.
They will see students with appointments. The clinic offers student physicals and immunizations for school aged children.
If you are interested, please print BOTH forms, fill them BOTH out and return them in person or by mail to

If you bring your forms to the Administration Center, you will receive a date and have an appointment scheduled at that time.
If you mail your forms back, you will be contacted to arrange an appointment time for your child.
Children entering Pre-kindergarten, Kindergarten and Sixth grade will be served first.
Appointments will be based on when the registration information is received in our office.
You will receive a confirmation letter in early August, 2008 reminding you of the time and day for your child's appointment.
